By submitting this membership registration, I acknowledge that I am enrolling as a member of Continuance Dance & Athletics (CDA) at our new facility.
CDA is committed to providing clear communication and support to all members. If any unexpected changes occur, we will notify members promptly and provide appropriate options as needed. Should there be a significant disruption to facility operations, members may request a refund of any advance payments.
Cancellation Policy:
Members may cancel their membership at any time by submitting written notice (email or signed letter) to [Continuance Dance & Athletics]. Cancellations require a minimum of 30 days’ notice prior to the next billing date.
Membership fees will continue to be billed and the membership will remain active during the 30-day notice period. After the notice period ends, no further charges will be applied.
Membership fees are non-refundable once the billing period has started. If notice is given less than 30 days before the next billing date, one final billing cycle will occur.
Unused Services:
Any unused services or sessions do not roll over and must be used within the month they are purchased. Services will expire at the end of each month and cannot be carried forward to the next month.
